About The Service
The Inclusion Service work with, support and coordinate services for children with additional needs and disabilities, their families and settings/schools to ensure that children access their entitlement with smooth transitions into a range of settings. We work in partnership with parents, education services, and other professionals in education and health.

The Inclusion Team monitors and supports schools/settings to successfully include all children with additional needs and disabilities to access their entitlement, environment and make progress.
About the job
An exciting opportunity has arisen for an ALN Transition Worker (Placements), working as part of the wider Inclusion Team within the Education Service and with partners in many different agencies. The role focuses on working with parents/carers ensuring that the transition into specialist provision is as smooth as possible for children with additional needs and disabilities. The successful candidate will focus on placements from early years into reception as well as our Complex Learning Needs special schools across Cardiff.
What We Are Looking For From You
We are looking for a highly organised, motivated and passionate individual who can join our dynamic forward-thinking team to improve outcomes for children through partnership working. The successful candidate must have excellent communication skills both verbal and written. They will need to be able to manage difficult conversations as part of disagreement resolution.

Internal candidates who wish to apply for this position on a secondment basis must obtain approval prior to application using Form SEC1 (4.C.081). Requests may only be approved by the relevant Director/ Assistant Director/ Chief Officer or senior nominated officer graded no lower than OM2 or in the case of schools based staff the Headteacher / Governing Body.

This post is full time and will be 37 hours per week across 52 weeks (non term time).
